Medical and economical efficacy evaluation of non-invasive prenatal fetal Rh factor screening

The objective. To assess the medical and economic efficacy of prenatal noninvasive screening for the foetal rhesus factor in rhesus-negative pregnant women. 

Patients and methods. A retrospective medical and economic analysis of the results of noninvasive diagnosis of the Rh factor of the foetus in 10 618 Rh-negative pregnant women was performed who underwent this examination in the molecular-genetic laboratory of the Samara Regional Medical Centre «Dynasty» over the period 2012–2017. Of them, 223 Rh-negative pregnant women were chosen as a complete sample to form a selective control group for determining the medical efficacy (sensitivity and specificity) of the method.

Results. The negative Rh factor of the foetus was found in 29.2% of cases (3100 women), which is not contradictory to the genetic laws of inheritance. The economic effect of introducing the method of prenatal diagnosis, calculated as a difference between the cost of immunoprophylaxis (without indications) and the cost of noninvasive diagnosis of the whole cohort of examined women was 11.04 mln roubles over the 5-year period. The medical efficacy was assessed by labour outcomes, a high sensitivity (98.5%) and specificity (96.6%) of the diagnostic method have been established. 

Conclusion. Prenatal screening might be recommended for wide-scale introduction in medical practice of obstetriciansgynaecologists for early noninvasive diagnosis of the rhesus factor of the foetus by blood of pregnant women as a method with a high medical cost-effectiveness.
