The role of gene polymorphisms of the renin-angiotensin system in the etiology of early and late preeclampsia

The objective. To study the incidence of the angiotensin-converting enzyme ACE gene I/D polymorphism and the angiotensin II type 1 receptor (AGTR1) gene A1166C polymorphism in women, whose pregnancy was complicated by severe early- and  late-onset preeclampsia.

Patients and methods. A retrospective case-control study was performed that included 41 pregnant women with preeclampsia (basic group) and 25 patients with uncomplicated pregnancy (control group). In the basic group, we singled out two subgroups – early and late preeclampsia. The first (15 patients) comprised cases, when delivery was performed before the 34th weeks of pregnancy due to a severe state of mother and/or fetus. The late preeclampsia group (26 patients) included variants of the development of this complication after the 34th week of gestation.

Results. The I/D polymorphism of the angiotensin-converting enzyme АСЕ gene is associated with a risk for development of PE. The presence of the D-allele enhances the risk of severe PE. In the DD genotype, a possibility of early PE is by 5 times higher than of late PE. We have obtained data that confirm involvement of the renin-angiotensin system and of its encoding genes in the development of PE. We can say that АСЕ gene polymorphism is a genetic predictor of early and severe PE. Investigation of АСЕ gene polymorphic loci permits to use these gene markers to assess an individual prognosis of the  development and specific course of PE.

Conclusion. The I/D polymorphism of the АСЕ gene is associated with a risk for development of PE. The presence of the D-allele enhances the risk for severe PE. In the DD genotype, a possibility of early PE is by 5 times higher than of late PE.
