Etiopathogenetic specificities of gestosis and placental insufficiency in pregnant women with arterial hypertension

The review deals with a topical obstetric problem – pregnancy developing on the background of arterial hypertension (AH). The  article presents modern data on the role of hormonal and immune statuses, the trace element and biochemical composition of blood in pregnant women. Basic points referring to the etiology and pathogenesis of gestosis and placental insufficiency (PI) are presented (immune disadaptation, placental ischemia, toxic effect of low density lipoproteins, genetic predisposition, endothelial dysfunction, occult and multigene forms of thrombophilia). Literature data on specific features of central hemodynamics in women with AH are summed up. Special attention is paid to impaired production of growth factors of peptide  nature, which leads to incomplete invasion of trophoblastic villa, development of endothelial dysfunction in the «motherplacentafetus» system, pathology of formation of PI components and gestosis.
